Types of aid to be reported include but are not limited to outside scholarships, departmental awards, VA benefits, tuition remission, tuition exchange, third party assistance etc.

If you have more than five awards, you can complete this form multiple times.

Please note the awards reported on this form will not show on your account with the Cashier’s Office until we have received the funds from the organization.
